One systematic five-letter protocol for every hyperkalemic patient entering the emergency department.

G

Gauge Severity

Treat the ECG, not just the lab value! An acute rise is far more lethal than a chronic high baseline. Evaluate for peaked T-waves, PR prolongation, QRS widening, or sine wave.

Rule out pseudohyperkalemia if ECG is normal & patient asymptomatic.

U

Use Calcium

Stabilizes the cardiac membrane resting threshold. Does NOT lower potassium. Calcium Gluconate 10% 10 mL IV over 2–3 min.

Effect lasts 30–60 min. Repeat in 5–10 min if wide QRS persists!

Administer Shifters

Drives potassium intracellularly temporarily. Regular Insulin 10 units IV + D50W 50 mL + Nebulized Salbutamol 10–20 mg.

Monitor glucose hourly for 4–6 hrs for delayed hypoglycemia!

Remove Potassium

The only step that actually eliminates potassium from the body. Loop diuretics (if kidneys work), GI binders (SZC/Lokelma, Patiromer), or emergent hemodialysis.

Shifting wears off in 4–6 hrs; removal prevents fatal rebound.

Determine Cause & Bed

Medication reconciliation (stop ACEi, ARB, spironolactone, NSAIDs, trimethoprim). Identify AKI, DKA, or rhabdomyolysis.

Continuous telemetry bed. Never discharge without confirmed falling trajectory.

Interactive electrical progression from mild elevation to pre-terminal peri-arrest sine wave.

STAGE 5: SINE WAVE PATTERN (PERI-ARREST, K⁺ > 8.0 mmol/L) 25 mm/s • 10 mm/mV

The pre-terminal pattern. Widened QRS merges seamlessly with the T-wave into an undulating biphasic sine wave. This is a peri-arrest finding preceding VF or asystole. Administer IV Calcium immediately!

1 Peri-Arrest

Never Wait for Confirmatory Labs in Sine Wave

If an unstable or altered patient has an ECG showing a sine-wave pattern or wide QRS, treat immediately with IV calcium and shifting agents while bloods are processing. Waiting for the lab leads to cardiac arrest in the scanner or triage bay.

2 Iatrogenic Trap

Always Recheck Glucose Repeatedly for 6 Hours

Delayed hypoglycemia is the single most common iatrogenic complication of hyperkalemia management. Insulin-induced hypoglycemia often peaks 2 to 4 hours post-administration, particularly in kidney injury where insulin clearance is impaired.

3 Rebound Clock

Anticipate the Rebound at 60 min and 4 Hours

Calcium membrane stabilization only lasts 30 to 60 minutes. Insulin/glucose intracellular shifting wears off after 4 to 6 hours. If elimination (loop diuretics, binders, dialysis) has not occurred, potassium will flood back into the circulation.

4 DKA Dynamics

DKA Serum K⁺ Hides Profound Whole-Body Deficit

In diabetic ketoacidosis, severe acidosis and insulin lack drive potassium out of cells, yielding normal or elevated serum readings despite massive whole-body urinary depletion. Once insulin therapy starts, potassium plummets precipitously; prepare to replace, not lower.

5 False Reassurance

Chronic Kidney Disease Tolerates Higher Numbers

End-stage renal disease patients on dialysis frequently maintain serum potassium levels of 6.0 to 6.5 mmol/L with minimal ECG distortion due to cardiac electrochemical adaptation. Conversely, an acute rise from 4.0 to 6.5 mmol/L in rhabdomyolysis or AKI triggers lethal arrhythmias rapidly.

6 The Sequence

Sequential Order Over Simultaneous Chaos

Do not chase the underlying cause (e.g. stopping lisinopril or ordering ultrasound) before stabilizing the myocardium with calcium and initiating shifting. Stick to the GUARD order: G → U → A → R → D every single time!
